Goal: The mission of Asistencia para Latinos is to empower the Latino community towards self-sufficiency through service, education, advocacy, and inter-agency collaboration. The Winds of Change is a movement developed by Asistencia para Latinos to address the issues of cultural isolation and language barriers presented by the new population of immigrants coming to the rural resort areas of Colorado.

Goal: Triple P aims to prevent maltreatment and behavioral, emotional and developmental problems in children by strenghtening the knowledge, skills and confidence of parents.
Impact: The program found a 25-35% reduction in child maltreatment, child maltreatment related hospitalizations and injuries, and foster home placements of children for young children at two-year followup of the program.

Goal: The goal of the Urban Mold and Moisture Program is to reduce environmental mold, moisture, and asthma triggers in homes to improve pediatric respiratory health.

Goal: The goal of this program is to reduce the risk of foodborne illness for Maricopa County citizens and visitors by using food sample results to address complaints and increase the efficacy of routine inspections.

Goal: The mission of the Pasadena Community Gardens Conservancy is to improve family health in urban food desert neighborhoods through grants for community gardens and nutrition education.
Impact: Pasadena Community Gardens Conservancy partnered with the City of Pasadena to establish the Villa-Parke Community Center, where community members can learn about gardening, cooking, nutrition.

Goal: The Wellness Curriculum was developed 1) to address the racial/ethnic disparities in health outcomes and 2) to fill the need for information that the inmates themselves expressed.

Goal: Shepherd's Center Central is a local leader in the regional effort to ensure that all people can age successfully with dignity, security, and respect.
Impact: The Wheels that Care program helped 77 older adults attend their necessary appointments and maintain their independence through 1,550 hours of travel time to and from participants' homes and their appointments.
